MD² International, LLC ("MD²", “we”, “us”) recognizes that your privacy and the protection of your personal information are important to you. This statement (“Privacy Policy”) discloses how MD² collects, uses, and discloses your personal information.  This Privacy Policy applies to information we collect when you access or use our websites (collectively, our “Services” or “Web sites”) or when you otherwise interact with us, such as on social media pages.  How MD²  and its affiliates may use and disclose medical information about you, including information provided through any online portal and other rights under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), are not covered by this Privacy Policy.

Collection of Information

Information You Provide to Us

MD² may collect personal information about you that you specifically and voluntarily provide. This information includes information that can identify you as a specific individual, such as your first name, last name, phone number, or e-mail address, the practice location you would prefer to visit, and any other information you choose to provide. On all such pages that collect this information, we specifically describe what information is required in order to provide you with the service or information that you have requested. MD² will not sell, share, or rent this information to others in ways different from what is disclosed in this statement.

Information We Collect Automatically

We automatically collect certain information about your interactions with us or our Services, including:

  • Device and Usage Information: We collect information about how you access our Services, including data about the device and network you use, such as your hardware model, operating system version, mobile network, IP address, unique device identifiers, browser type, and app version. We also collect information about your activity on our Services, such as access times, pages viewed, links clicked, and the page you visited before navigating to our Services.
  • Information Collected by Cookies and Similar Tracking Technologies: We (and those who perform work for us) use tracking technologies, such as cookies and web beacons, to collect information about your interactions with the Services. Cookies are small data files stored on your hard drive or in device memory that help us improve our Services and your experience, see which areas and features of our Services are popular, and count visits. Web beacons (also known as “pixel tags” or “clear GIFs”) are electronic images that we use on our Services and in our emails to help deliver cookies, count visits, and understand usage and campaign effectiveness.

 MD² may collect certain technical information from your computer each time you request a page during a visit to MD². This information is collected from your computer’s Web browser and may include your IP address, operating system, Web browser software (e.g. Chrome, Safari or Internet Explorer), screen resolution, and referrer Web site. Wireless users: Please check with your wireless company if you are using your cellular phone to access the Internet to understand whether such personal information may be used to identify you.

Information We Collect from Other Sources

We obtain information from other sources. For example, we may collect information from advertising networks and data analytics providers. This information includes your age or approximate age and other demographic information.

Cookies and Other Web Technologies

A cookie is a text file that is placed on your hard disk by a Web page server. Cookies cannot be used to run programs or deliver viruses to your computer. Cookies are uniquely assigned to you, and cookies set by MD² can only be read by MD². One of the primary purposes of cookies is to provide a convenience feature to save you time. You have the ability to accept or decline cookies. Most web browsers automatically accept cookies, but you can usually modify your browser setting to decline cookies if you prefer. Personal information collected on this site may be stored and processed in the United States or any other country in which MD² or its affiliates or agents maintain facilities, and by using this site, you consent to any such transfer of information outside of your country.

Please be aware that MD², Inc. is not responsible for the privacy practices of such other sites. MD² is not responsible for the privacy statements or other content on Web sites outside of the md2.com domain. This privacy statement applies solely to information collected by this Web site.

Sharing Your Information

We share personal information in the following circumstances or as otherwise described in this policy:

  • We share personal information with vendors that access personal information to perform work for us, such as companies that assist us with web hosting, fraud prevention, marketing and advertising.
  • We may disclose personal information if we believe that disclosure is in accordance with, or required by, any applicable law or legal process, including lawful requests by public authorities to meet national security or law enforcement requirements.
  • We may share personal information if we believe that your actions are inconsistent with our user agreements or policies, if we believe that you have violated the law, or if we believe it is necessary to protect the rights, property, and safety of MD2, our users, the public, or others.

We also share aggregated or de-identified information that cannot reasonably be used to identify you.

MD² does not sell or rent your personal information to anyone. Except as otherwise described in this Privacy Statement, personal information you provide on this site will not be shared outside of MD² and its affiliates without your notification and permission. We may partner with another party to provide specific services. When the user signs up for these services, we will share names or other contact information necessary for the third party to provide these services. These parties are not allowed to use personally identifiable information except for the purpose of providing these services.

Control Over and Access to Your Information

When you give us personal information, MD² will not share that information with third parties without your permission other than for the limited exceptions already listed. It will only be used for the purposes stated above.

ADVERTISING AND ANALYTICS

We engage others to provide analytics services and serve advertisements across the web and in mobile apps. These entities may use cookies, web beacons, device identifiers, and other technologies to collect information about your use of our Services and other websites and applications, including your IP address, web browser, mobile network information, pages viewed, time spent on pages or in mobile apps, links clicked, and conversion information. This information may be used by MD² and others to, among other things, analyze and track data, determine the popularity of certain content, deliver advertising and content targeted to your interests on our Services and other websites, and better understand your online activity.

We also work with third parties to serve ads to you as part of customized campaigns on third-party platforms (such as Instagram). As part of these ad campaigns, we or the third-party platforms may convert information about you, such as your email address and phone number, into a unique value that can be matched with a user account on these platforms to allow us to learn about your interests and serve you advertising that is customized to your interests. Note that the third-party platforms may offer you choices about whether you see these types of customized ads.

YOUR CALIFORNIA PRIVACY RIGHTS

The California Consumer Privacy Act or “CCPA” (Cal. Civ. Code § 1798.100 et seq.) [and the Shine the Light law (Cal. Civ. Code § 1798.83)] afford[s] consumers residing in California certain rights with respect to their personal information. If you are a California resident, this section may apply to you.

California Consumer Privacy Act

In the preceding 12 months, we have collected the following categories of personal information:

(A) Identifiers such as a real name, alias, postal address, unique personal identifier, online identifier, internet protocol address, email address, account name, social security number, driver’s license number, passport number, or other similar identifiers.

(B) Internet or other electronic network activity information, including, but not limited to, browsing history, search history, and information regarding a consumer’s interaction with an internet website, application, or advertisement.

(C) Geolocation data.

(D) Audio, electronic, visual, thermal, olfactory, or similar information.

(E) Inferences drawn from any of the information identified in this subdivision to create a profile about a consumer reflecting the consumer’s preferences, characteristics, psychological trends, predispositions, behavior, attitudes, intelligence, abilities, and aptitudes.

While we do not provide personal information about you to other companies for money, we do allow our data analytics providers to collect information about you as further described in the Advertising and Analytics section of this Privacy Policy.

Subject to certain limitations, you have the right to (1) request to know more about the categories and specific pieces of personal information we collect, use, and disclose, (2) request deletion or correction of your personal information, (3) opt out of any “sales” or “sharing” of your personal information that may be occurring, and (4) not be discriminated against for exercising these rights. Data Retention

We store personal data associated with your account for as long as your account remains active. If you close your account, we will delete your account data within 60 days; otherwise, we will delete your account data after one year of inactivity. We store other personal data for as long as necessary to carry out the purposes for which we originally collected it and for other legitimate business purposes, including to meet our legal, regulatory, or other compliance obligations.

Changes to this Privacy Statement

We may update this Privacy Statement. When we do, we will also revise the "last updated" date at the top of the Privacy Statement. For material changes to this Privacy Statement or how MD² will use your personal information, we will notify you either by placing a prominent notice on the home page of our Web site or by directly sending you a notification. We encourage you to periodically review this Privacy Statement to stay informed about how we are protecting the personal information we collect.
