MD² may collect personal information about you that you specifically and voluntarily provide. This information includes information that can identify you as a specific individual, such as your first name, last name, phone number, or e-mail address, the practice location you would prefer to visit, and any other information you choose to provide. MD² may collect certain technical information from your computer each time you request a page during a visit to MD². MD² does not sell or rent your personal information to anyone. ADVERTISING AND ANALYTICS

YOUR CALIFORNIA PRIVACY RIGHTS
The California Consumer Privacy Act or “CCPA” (Cal. Civ. Code § 1798.100 et seq.) [and the Shine the Light law (Cal. Civ. Code § 1798.83)] afford[s] consumers residing in California certain rights with respect to their personal information. If you are a California resident, this section may apply to you.
